This form may be completed by any student, vendor, visitor, or employee of LMU to report an alleged violation of LMU’s Title IX/Sexual Harassment Policy. The University’s Title IX/Sexual Harassment policy prohibits discrimination based on sex, including but not limited to sexual harassment such as sexual assault, domestic violence, dating violence, and stalking. While as much information as possible is appreciated, you may still submit this form if you do not have all of this information.This link may be used to report anonymously.

However, please note that, per federal law, the information included in this form is not confidential, but will be kept as private as possible in effort to protect individuals and the LMU community.
