Effective with the Fall 2008 semester, Lincoln Memorial University is requiring that all undergraduate and masters level students, taking six or more credit hours, have health insurance coverage. Students may provide proof of their own insurance that is equivalent to the coverage of the LMU plan or subscribe to the LMU plan.  Student accounts will be automatically charged for the new insurance plan unless proof of insurance is provided on or before:
  • Fall '08 Waiver Deadline August 29, 2008
  • Spring '09 Waiver Deadline January 29, 2009
  • Summer '09 Waiver Deadline June 6, 2009

Student health insurance policies must meet the following criteria to obtain a waiver:          

  1. A maximum benefit per sickness or injury of $50,000 or higher            
  2. An annual deductible of $1500 or less            
  3. Coverage at 80% in network or 50% out of network, or higher            
  4. A plan year maximum for prescription drugs of $1500 or higher            
  5. The plan must meet all Federal and State insurance requirements and claims must be paid within the United States
  6. Network Providers must be available within the region

PROCESS FOR WAIVER REQUEST 

  • Students must fax or bring a copy of their insurance card and an explanation or summary of insurance benefits to the Office of Student Services, Student Center, Room 308, FAX 423.869.6204, that reflects that the policy meets the criteria listed above.
    • Students who have TennCare insurance or an equivalent state policy or those with military insurance need only to submit a copy of their current insurance card to the Office of Student Services.
    • LMU employees should submit a copy of their insurance card.  Employee IRS dependents only need to supply a memo from the Human Resources Office stating that they are covered under the LMU Employee medical insurance plan.
  • If the student's name is not on the insurance card, the student should: 1) get a letter from their insurance company or Human Resources Department verifying that they are covered under the plan; or 2) Go to carrier website and print off proof that their coverage is active.
  • If the plan meets the required criteria, the charges for the insurance plan will be removed from the student account.
  • The student should allow at least ten business days for the credit to be processed after the information has been sent to the Office of Student Services.
  • All notifications of waiver approvals will be sent via the student's LMU email account.  It is the student's responsibility to check their email accounts for approval notice.  If an LMU email account has not been issued, then students should contact:  Lesley Chumley, 423.869.7166, or by email at lesley.chumley@lmunet.edu, to follow up on the status of the waiver.
NOTE: University personnel and insurance company personnel will not have access to personal health information through the waiver process. No personal health information is required or needed in the waiver process.

ON-CAMPUS HEALTH CARE SERVICES
A special feature of the plan will be no out-of-pocket costs for office visits for students who receive care at the LMU-DeBusk College of Osteopathic Medicine, Outpatient Services department located on the main campus. For those who choose not to use this service, there is an affordable office co-pay and annual deductible. The plan is also accepted at many area health care providers.
